Enrollment forms may be downloaded below, or picked up in the preschool, school or daycare offices.

Daycare Forms Daycare Enrollment Forms State Required Enrollment State Required Medical State Medical Physical Care Plan	You may download and print these forms to fill out for enrollment. All forms to the left must be completed in order for your enrollment packet to be complete. This form needs to be filled out if you checked the box on the State required form.
Preschool Forms Registration Form Preschool Enrollment Forms State Required Enrollment State Required Medical State Medical Physical Care Plan Preschool Parent Handbook	This form and the registration and supply fee are needed for registation to be complete. You may turn this form into the preschool office only. These forms will be available spring 2010. . This form needs to be filled out if you checked the box on the State required enrollment form. This is the parent handbook for the preschool.
Tuition Direct Debit Forms Tuition Express Cypress Letter and Form Bank Draft Application	These forms are for enrollment in our Tuition Direct Debit system. To enroll, all forms must be completed with a voided check. We are currently only offering the bank account funds transfer.
Other Forms State Required Children's Medical Statement	This is due every year and is good for 13 months. This form must be completed and signed by the doctor. A copy of the shot record alone will not meet the state requirement.
Sleep Position Waiver	We must have this and a note from the doctor if you would like your child to sleep in any position other than on their backs.
Medication Administration Form	This form is needed if you have medicine that you would like us to give your child. If it is an over the counter medicine, there must be a dosage on the box or bottle for your child's age or weight. All medicine must be in the original container.
